PN-U14 / Visual concept
A reassuring saturation does not measure carbon-dioxide removal.
A postoperative adult receives prescribed oxygen. Verified observations change between 09:00 and 10:00. The urgent team is being activated, and the PN assists within the assigned role.
PN-U14 / Worked case
SpO₂ increases from 93% to 97%: four percentage points. This observation does not measure carbon-dioxide removal.
Breathing becomes shallow at 8/min, responsiveness declines and PaCO₂ rises from 42 to 60 mmHg. The changes do not cancel one another.
Communicate times, depth/rate, responsiveness, oxygen setting and relevant medication exposures. Support the authorized airway/breathing response and reassess its effect.
Neither follows. Ventilation and alertness worsened; timing alone does not establish the cause. Medication or ventilation changes require the responding team’s authorized plan.
Fictional teaching case. A pulse oximeter does not measure CO₂, establish sufficient hemoglobin or prove adequate tissue perfusion. Verify questionable readings without delaying help.
A practical study routine
Use the guide actively: make a decision before checking the answer, then bring the lesson into your next attempt.

Step 2
Select an appropriate response using the scenario’s authority and facts.
Step 3
Review why reassessment, reporting, or escalation is needed.

The medication example checks the chosen volume against the ordered amount. I like that the explanation still keeps the other safety checks visible; getting the arithmetic right doesn't settle the whole medication decision. The backward check also makes the tempting wrong volume easy to understand: it would deliver an amount different from the order.
The follow-up case identifies the unsafe step and checks a fresh scenario. That's a clear way to show why a person's agreement alone isn't enough evidence that the teaching has been understood.
The postoperative example puts improving saturation beside worsening breathing and responsiveness. The comparison gives a clear reason to look at oxygenation and ventilation separately instead of letting one improving number settle the whole picture.
The low-glucose case changes while the team is getting ready to act. I appreciate the explanation of how alertness affects the oral route, with the need for urgent assistance kept within the PN role.
The circulation example follows urine output, pulse and the same daily activity over time. That makes the comparison useful: unchanged swelling doesn't automatically outweigh new concerns in the other observations.
